CSI SPORTS MEDICINE ATHLETE PAPERWORK INFORMATION AND INSTRUCTIONS 
The following information is what each athlete needs to know about the medical forms they will be receiving from the CSI Sports Medicine Staff.   
 
Please complete and return the forms as early as possible so we have adequate time to review and approve them before any athletic participation at CSI is allowed. We frequently encounter errors or information that requires clarification, so submitting your forms sooner helps avoid delays. 
All forms must be submitted by June 30, 2026, to allow sufficient time for review before your season, including preseason activities. Forms received after June 30, 2026, may delay your clearance, and we cannot guarantee participation eligibility for the first day of team practice. 
It is recommended that the paperwork be completed on a computer rather than a phone or tablet. 
PHYSICAL:  
This form is linked below and must be printed and completed by an MD, DO, NP, or PA. Forms completed by physical therapists, chiropractors, or other healthcare providers cannot be accepted. Only physical dated after May 1, 2026 will be accepted. Athletes do need to sign their name at the bottom of the form. The completed form may be returned by email or submitted as a hard copy. Please note that we are only able to accept the official CSI Physical Form and will not accept any other version.

CSI SPORTS MEDICINE ATHLETE PACKET: 
This packet will be sent through Adobe and must be completed and electronically signed online. Once all required information has been submitted, the form will automatically be returned to us via Adobe. If you are 18 years of age or older, this must be completed and signed by you the athlete; no other signatures will be accepted.  
 
Please provide as much detailed information as possible to help avoid delays or the need for follow-up communication for clarification. Be sure to include dates, specific injuries, surgeries, treatments, and any other relevant medical history when applicable. 
If you are under the age of 18, your parent or guardian must complete and sign the paperwork. Please do not open the packet yourself; instead, forward the email directly to your parent or guardian. If we already have a parent or guardian email address on file, the packet will be sent directly to them. 

INSURANCE POLICY FORM: 
This form will also be sent through Adobe and must be completed and electronically signed by the primary health insurance policy holder, which in most cases is a parent or guardian. Please forward the email directly to the primary policy holder without opening the document yourself. If you have your own health insurance policy, you will need to complete and sign the form personally. 
 
All CSI athletes are required to have a primary health insurance policy to participate in athletics. 
Once all required information has been submitted, the form will automatically be returned to us via Adobe. Please also email a clear photo or copy of both the front and back of your insurance card to us at athletictraining@csi.edu
 
FAQs:
  • Do I need to re-do the paperwork if I completed it last year? Yes, all athletes must complete all the CSI Sports Medicine Athlete Paperwork annually. 
  • Do I have to get tested for sickle cell trait? No, it is not required, but it is recommended, especially if you are in the high-risk population.
  • Does my physical exam need to be completed in Idaho? No, as long as the physical is completed on the CSI Physical Form, it can be done anywhere.
  • Do I have to print my physical? Yes, it needs to be printed in order for the approved medical professional and yourself to sign.
