Southern Idaho Baseball Camp - Registration Form
Please complete this registration form and mail it to the address listed below. Registration must be recieved on or before December 23, 2008. Registration is limited to 200 participants and will be on a first come - first serve basis if cancellations occur. This form may be duplicated to register additional players (Ages 12-18).
